Aspiration during Monitored Anesthesia Care
نویسنده
چکیده
To the Editor: An important study by Savilampi et al.1 demonstrating the rate of pulmonary aspiration in adults undergoing monitored anesthesia care with remifentanil was recently published in AneStheSiology. There is an important limitation of the study worth considering when interpreting the study results. The method of aspiration detection does not differentiate between pharyngeal-to-pulmonary aspiration (either oropharyngeal or nasopharyngeal) and gastric-to-pulmonary aspiration. A radionuclide-labeled solution was introduced into the nasopharynx during the study period; therefore, it is not clear whether its detection in the thorax represents aspiration of nasopharyngeal/oropharyngeal secretions, gastric contents, or both. The importance of this point is that gastric-to-pulmonary aspiration (via macroaspiration or gastroesophageal reflux disease) has been implicated in the development of aspiration pneumonitis, pneumonia, and acute respiratory distress syndrome,2–5 whereas aspiration of oropharyngeal secretions may contribute to the development of pneumonia but not necessarily pneumonitis or acute respiratory distress syndrome (other than acute respiratory distress syndrome secondary to pneumonia).6

Three Cases of a Rare Association: Double Aortic Arch
Background Vascular ring is less than 1% of congenital heart disease. Double aortic arch (DAA) is the most common form of it. Its detecting is important because of the effects of pressure on the esophagus and trachea. Case Report In this study, three children suffering from double aortic arch with symptoms of dysphagia and recurrent aspiration, which in two cases had led to cardiac arrest, wer...
متن کاملLow incidence of pulmonary aspiration in children allowed intake of clear fluids until called to theatre
Background: International guidelines recommend two hours of clear fluid fasting prior to general anesthesia. The pediatric anesthesia unit of Uppsala University Hospital implements a more liberal fasting regime for more than a decade; thus children scheduled for elective procedures are allowed to drink clear fluids until called to theatre. We hypothesized that this practice would not increase t...
متن کاملDexmedetomidine use during epiduroscopy reduces fentanyl use and postoperative nausea and vomiting: A single-center retrospective study
Objectives Minimally invasive epiduroscopy has recently been reported as an effective treatment procedure for chronic and intractable low back pain. However, no study has determined safe anesthetics for monitored anesthesia care during epiduroscopy. We aimed to compare and evaluate conventional monitored anesthesia care drugs with dexmedetomidine. Methods A retrospective study including all p...
متن کاملOutcome of Carotid Endarterectomy after Regional Anesthesia versus General Anesthesia - A Retrospective Study Using Two Independent Databases
BACKGROUND Carotid endarterectomy (CEA) is effective in reducing stroke risk in selected patient groups. The ideal anesthetic technique remains controversial in light of literature between general anesthesia (GA) and regional anesthesia (RA) for CEA. METHODS We studied the NSQIP data from 2005 to 2012. There were 32,718 patients receiving general anesthesia (GA) and 5,384 patients receiving r...
متن کاملComparison of Anesthetic Techniques on Outcomes of Pediatric Rigid Bronchoscopy for Foreign Body Removal
Background Although both methods of spontaneous respiration and controlled ventilation during anesthesia are safe and effective for managing children with foreign body aspiration, there is no consensus from the literature as to which technique is optimal. This study aimed to determine the outcomes of anesthetic techniques in pediatric rigid bronchoscopy for foreign body removal. Materials and M...
